Skin Form

Medical release form REQUIRED by MSHSAA for wrestlers to participate with skin lesion(s). The form must be completed by a physician, and must be presented to the official at weigh-ins. Failure to provide the completed form will result in your child not being able to wrestle at that meet or tournament. The purpose is to (1) protect wrestlers from exposure to communicable skin disorders. (2) Allow wrestlers to participate as soon as it is reasonably safe for them and for their opponents and/or teammates using the same mat.

Packet of paperwork required by Francis Howell High School before a wrestler can practice or compete. Multiple Forms including (1) Pre-Participation Physical Form, (2) Emergency Form for Athletic Trainer, (3) Parental Consent to Participate Form, (4) Academic Eligibility Standards form, (5) District Activities Handbook Acknowledgement Form, (6) Impact testing Consent Form, (7) Private Transportation Permission Form. Paperwork can be turned in at Winter Meet the Coaches Night or any time prior to first day of practice.
